Deteriorated Parts Allowed Flutter Which Led to Fatal Crash at 2011 Reno Air Races
WASHINGTON - The National Transportation Safety Board determined today that deteriorated locknut inserts found in the highly modified North American P-51D airplane that crashed during the 2011 National Championship Air Races in Reno, Nevada, allowed the trim tab attachment screws to become loose, and even initiated fatigue cracking in one screw. This condition, which resulted in reduced stiffness in the elevator trim system, ultimately led to aerodynamic flutter at racing speed that broke the trim tab linkages, resulting in a loss of controllability and the eventual crash.
On September 16, 2011, as the experimental single-seat P-51D airplane The Galloping Ghost, traveling about 445 knots, or 512 mph, in the third lap of the six-lap race, passed pylon 8, it experienced a left-roll upset and high-G pitch up. During the upset sequence, the airplane's vertical acceleration peaked at 17.3 G, causing incapacitation of the pilot. Seconds later, a section of the left elevator trim tab separated in flight. The airplane descended and impacted the ramp in the spectator box seating area, killing the pilot and 10 spectators and injuring more than 60 others.
In Reno, the fine line between observing risk and being impacted by the consequences when something goes wrong was crossed, said NTSB Chairman Deborah A. P. Hersman. The pilots understood the risks they assumed; the spectators assumed their safety had been assessed and addressed.
Contributing to the accident were the undocumented and untested major modifications made to the airplane, as well as the pilot's operation of the airplane in the unique air racing environment without adequate flight testing.
The nearly 70-year-old airplane had undergone numerous undocumented modifications. The modifications, designed to increase speed, included shortening of the wings, installation of a boil-off cooling system for the engine, increasing the elevator counterweights, modification of the pitch trim system, and changing the incidence of the horizontal and vertical stabilizers.
Although the Federal Aviation Administration required that a flight standards district office be notified in writing of any major changes made to The Galloping Ghost before it could be flown, investigators could find no records that such notifications were made except for the installation of the boil-off cooling system. The undocumented major modifications were identified through wreckage examinations, photographic evidence, and interviews with ground crewmembers.
In April, while the investigation was ongoing and after the NTSB's investigative hearing in January on air race and air show safety, the NTSB issued 10 safety recommendations to the Reno Air Racing Association, the National Air racing Group Unlimited Division, and the FAA. These recommendations addressed:
requiring engineering evaluations for aircraft with major modifications;
raising the level of safety for spectators and personnel near the race course;
improving FAA guidance for air race and course design;
providing race pilots with high-G training and evaluating the feasibility of G-suit requirements for race pilots; and
tracking the resolution of race aircraft discrepancies identified during prerace technical inspections.
Although no additional safety recommendations were issued today, the Board reclassified nine existing recommendations as described below:
Eligibility Requirements for Aircraft with Major Modifications - recommendations A 12 9 and A-12-13 classified Open—Acceptable Response
Prerace Technical Inspection Discrepancy Tracking - recommendation A 12 10, classified Closed—Acceptable Action
Spectator Safety - recommendations A 12 14 and 15, classified Closed—Acceptable Action
High G Training, G-Suit Feasibility for Pilots - recommendations A 12 11, -12, -16, and -17, classified Closed—Acceptable Action
A tenth safety recommendation, issued to the FAA, which addressed air race and course design guidance was reclassified as Open—Acceptable Response on July 25, 2012.
It's good news for the air races that so many of our recommendations have been addressed, said Chairman Hersman. We will continue to push for the full implementation of all of our safety recommendations.

Reno is a city in the U.S. state of Nevada, located in the western part of the state, approximately 22 miles from Lake Tahoe. Known as The Biggest Little City in the World, Reno is famous for its hotels and casinos. It is the county seat of Washoe County, in the northwestern part of the state. The city sits in a high desert at the foot of the Sierra Nevada and its downtown area (along with Sparks) occupies a valley informally known as the Truckee Meadows. It is named after slain Union Major General Jesse L. Reno.
Reno is the third-most populous city in Nevada after Las Vegas and Henderson and the most populous city in the state outside the Las Vegas Valley. Before the late 1950s, Reno was the gambling capital of the United States, but Las Vegas' rapid growth, American Airlines' 2000 buyout of Reno Air, and the growth of Native American gaming in California have reduced its business. Older casinos were torn down (Mapes Hotel, Fitzgerald's Nevada Club, Primadonna, Horseshoe Club, Harold's Club, Palace Club), or smaller casinos like the Comstock, Sundowner, Golden Phoenix, Kings Inn, Money Tree, Virginian, and Riverboat were either closed or were converted into condos.
Because of its location, Reno has traditionally drawn the majority of its California tourists and gamblers from the San Francisco Bay Area and Sacramento, while Las Vegas has historically served more tourists from Southern California and the Phoenix area.
Several local large hotel casinos have shown significant growth and have moved gaming further away from the downtown core. These larger hotel casinos are the Atlantis, the Peppermill and the Grand Sierra Resort. The Peppermill was chosen as the most outstanding Reno gaming/hotel property by Casino Player and Nevada magazines. In 2005, the Peppermill Reno began a $300 million Tuscan-themed expansion.

The Great Reno Balloon Race, the largest free hot air ballooning event in the country, is held the first weekend after Labor day each September at Rancho San Rafael Regional Park in Reno, Nevada. The GRBR draws in a crowd of 140,000 on average each year, making it the largest free event in Northern Nevada.
The mission of The Great Reno Balloon Race is to provide a premier, safe, family-oriented, tourist-attractive, visually dramatic event that celebrates the joy of flight while remaining free. This mission has withheld the strains of time and a shifting economy yet remains free and larger than ever today.
History
The first Balloon Race was in 1982, when a mere 20 balloons graced the crisp Nevadan sky. Today, upwards of 100 balloons participate each year. An event that was first started to create a tourist attraction between the Nevada State Fair and the Reno Air Races has become the icon of the community. Bringing people from all over together who share a love for family, community and ballooning, The Great Reno Balloon Race has become a beloved tradition which is embraced by locals and tourists alike.
One of the most spectacular shows over the three-day event is the Great Reno Balloon Race is Dawn Patrol. A term patented by the board of trustees when Federal Aviation officials approved predawn flying regulations in 1978. Safety precautions such as special navigation lights became the status quo and The Great Reno Balloon Race quickly became a national role model for pre-dawn events in 1990 when it began launching balloons before sunrise.
During Dawn Patrol, pilots face unstable, pre-dawn, air conditions as their balloons illuminate the early morning darkness. Adding warmth to the cool morning, Dawn Patrol marks the beginning of the Balloon Race.
After eight years of the Dawn Patrol, yet another attraction was added to thrill onlookers as they waited for the show to begin. Just before Dawn Patrol, pilots light their lanterns for the Glow Show. The balloons take on an almost fictional appearance, not unlike a paper lantern which marks the beginning of the Balloon Race.

MARIA ROMAN ASSAULTED IN RENO NEVADA AUGUST 22 2010
On August the 22nd while visiting Reno, NV I attended a popular nightclub downtown Reno... Not long after I arrived people began approaching questioning my gender... most of course were men...... with the typical line my friends says you're a not a woman is that true? I usually give no reply since it's not their business, I must say it happens to me often...So I ignored it and continued about my business... however it seen like in this case it spread like wildfire...
A few hours later while I was waiting in the line to the woman's restroom... With no warning, and unprovoked I was blindsided by a blow to my left eye by a woman...this followed by others individual punches that seem to be coming for different directions ... I was pulled by my hair and fell to the ground I began to be kicked with the occasionally you fucking fag this is the woman's bathroom...
As this brutal attack continued all I can do is cover my face waiting for someone to make it stop... I was caught of guard I could not even defend my self. Finally the women and individuals stopped... It seemed like an eternity... as I picked my self from the floor I realized security guards, had been standing around while I was being beaten.
I was so confused and scared I ran out side so afraid not knowing who and how many people had attached me...My eye was almost closed shut... ones outside I could not contain my tears... as I exited the club the police and other security guards are standing around... I could barely speak just from how emotional I was... I told them I was just beaten in the club and that security did nothing, they witness my attack but dint detain the individuals or come to my assistance...Their blank stares were not a good sign. well it all went down from there...
Officer Michael L. Mahaffey asks what you want me to do. Do you know who hit you? When I told them. I believe it was a group of women they began laughing... one of them said. as big as you are some woman hit you after their remarks... I again told Officer Michael L. Mahaffey that security were present when I was attacked and did nothing. I wanted a formal report of my attack... I also told them that I had been called a fag multiple times during the attack which constitutes a hate crime in several states... at that moment I had no idea of the laws in Nevada however I wanted something tangible of what had happened to me at that club...he basically refused to make a report...
I was so infuriated by his disregard to what had just happened to me.... In a nutshell we exchanged some words ... not long after that I was told to put my hands behind my back... I was thrown against the police car and hand cuffed... All along his intention was to show me that I knew nothing† and should shut my mouth when told... because he said so... I figure I would at least give him a word or two And I might have thrown a couple of Mother Fuckers and Fuck You'S...
I was overwhelmed with the thought of how unfair life can be, how we fight all our lives to be who we are. How we are spit, on, beaten by a mob, treated like second class citizens yet expected to walk away with a smile on our face while replying WELL YES SIR...
When we march together as one community with a rainbow of diversity our message is stronger and clearer... I continue to take the oath to my self that I will do anything and everything in my power to expose and speak about the bigotry, hate and perpetuate apathy to hate crimes associated with gender, sexual orientation and all whom don't fit within the so called norm
keep me in your prayers...I filled a report once released regarding my assault currently in the detectives division...
The police department would benefit from knowing how large the community is, let them know that to be further victimize after an assault is unacceptable,
