Dermal fillers for tear troughs including live demonstration
Hi my name is Sandy Fieldhouse, I'm one of the doctors at The Victorian Cosmetic Institute and today I'm going to talk about treatment to the tear trough area. The tear trough area is the area just under the eye which often suffers from a loss of volume, this can be caused either through genetics or just through a loss of tissue or fat or muscle or drop from the cheek; and it is not an uncommon problem.
To treat the tear trough area we use dermal fillers. Dermal fillers can also be used in the cheeks, the lower face, the lips and a variety of other areas. The dermal filler acts to actually just fill the space, the dermal fillers that are widely used at this practice are based on a substance called hyaluronic acid. Hyaluronic acid is naturally occurring in the body and everybody's hyaluronic acid is the same, making it a very safe and stable product to use for filler.
It's a very straight forward procedure that takes about 30 to 45 minutes; we use a simple cannula technique with no sharp needles to put the filler in place. Treatment for this area will last usually anywhere from 6 months to 1 to 2 years. There is typically minimal discomfort associated with the procedure but a local anaesthetic can be used as can laughing gas if there's any discomfort involved. The procedure is usually associated with minimal side effects there can be some bruising and some swelling which may last for a few days.
As with all the dermal fillers we use if the result is unacceptable it can be reversed if necessary. This can be done very simply with a few small injections with an enzyme that actually dissolves the filler, this takes effect over a day or so and usually corrects any of the small number of problems we might have. You may find at the time of treatment that you look and you feel that the area is slightly under treated, that you can still see the tear trough that you came in to have corrected. This is completely normal, we like to slightly under correct the area because we know over the next 2 to 4 weeks that some fluid will be absorbed by the filler and that will then give us the result that we like.
So we're just about to commence the treatment now, as you can see Somaiyeh is quite relaxed, we're just going to have a press here; and we just very gently put the filler in. You can actually see that lifting as we go. So you can see we've corrected on this side and you can see that's just lifted the hollow she had in that area to a very natural, soft way and we're going to now do this side here. Using the exactly the same technique, using very fine threads of filler to gently fill that area. We often give it just a little gentle massage; it's a very soft product.
So you can see after the procedure the area that was hollow and quite pronounced before has a much softer, natural look. So now lets look at some before and afters. Before you can see the relative deficit or loss of volume in the under eye area. After the procedure you can see the area has been filled giving a very soft and natural result to the eye area.

The families of five Australian cyclists injured after being hit by a car during a training ride in Germany visited their loved ones in hospital on Wednesday.
One team member, 29 year old Amy Gillett, was killed in Monday's accident.
The six member squad of the Australian Institute of Sports (AIS) was training for this week's Tour of Thuringen race, which was due to begin on Tuesday.
Two squad riders, Louise Yaxley and Alexis Rhodes, are in a critical situation at the intensive care unit of the Friedrich Schiller University Clinic in Jena, in the east German state of Thuringia.
Doctors say they face at least a month in the hospital.
They were flown to Jena by helicopters after first being treated at separate hospitals in Leipzig and Zwickau.
Two other riders, Katie Brown and Kate Nichols, suffered multiple fractures but were in a less serious condition and could be released in two weeks, doctors said.
The fifth injured cyclist, Lorian Graham, remained hospitalised in Gera in a stable condition and is expected to be moved to Jena later on Wednesday.
The accident happened late on Monday when an 18-year-old driver lost control of a car near Zeulenroda, south of Leipzig, and drove into the squad during a training session.
The race will now start on Wednesday, with what would have been stage two, and run through Thuringia state before finishing on Sunday.
Amy Gillett, who represented Australia in rowing at the 1996 Atlanta Olympics and later represented Australia in international cycling events, had been training in Europe with the AIS squad.

The Streets Barber Stories - Episode 5 : Aaron
Each day when I cut hair on the streets I meet amazing people with amazing stories. Stories that most people never get to see or hear. Stories that can warm my heart, and break it at the same time.
This is Aaron. What a positive and great guy! Back in the day, I used to always see him around in Footscray, always saying hi and showing love when I was on the streets. I never managed to give him a cut, he would be busy or on the way somewhere, but would always say next time!. Recently, I had bumped into him and his attitude to life seemed completely changed, so different from the last time I saw him on the streets, he seemed to radiate positivity.
So, we made plans to hang out for a day in Footscray and for me to give him a cut. He lived there for 11 years, having recently moved to a more affordable and stable house for his partner and family. Footscray was a big part of his recovery with substance abuse, having moved here from Brisbane with his partner when they decided to get a way from the scene that had overtaken his life at the time. Aaron has battled with heavy drugs for over a decade, and would always manage to find a reason to relapse. Now he has managed to overcome his struggles and is moving forward with his life.
We walked around the streets of Footscray, reminiscing good and bad times, and how he dealt with his challenges. Aaron's story is powerful, and speaks about the importance of the right support around you during tough times.
