Health chiefs failed to act upon guidelines and kept the public in the dark over potentially deadly infections at Glasgow’s super hospital.
Pressure is growing on Health Secretary Jeane Freeman and NHS Greater Glasgow and Clyde to explain why details of a further outbreak at the Queen Elizabeth University Hospital were only revealed during a parliamentary quizzing session this week – seven days after medics determined it was of serious risk to the public.
A patient at Glasgow's Queen Elizabeth University Hospital is seriously ill after contracting a fungal infection.
Health Secretary Jeane Freeman said the patient was one of two affected by the infection, related to mucor mould.
It follows the death of a 10-year-old boy who contracted the cryptococcus infection, related to pigeon droppings.
Ms Freeman has ordered a review to investigate how the building's design, handover and maintenance contributed to effective infection control.
A forensic review into the crisis-hit Queen Elizabeth University Hospital could take months to carry out and may never uncover the reasons for three serious infections in patients.
Nicola Sturgeon faced questions over the outbreak of infection at Queen Elizabeth University hospital at First Minister's Questions.
Both Jackson Carlaw and his Labour counterpart Richard Leonard questioned the First Minister on the outbreak of Cryptococcus - an infection linked to pigeon droppings - that contributed to the death of a child at the Queen Elizabeth University Hospital in Glasgow.
The hospital, which was the largest in Europe when it opened in 2015, was a flagship project for the Scottish Government.

Half of Scotland's hospital laundries face the axe in plan to save £2.7m
PLANS to slash the number of NHS laundries in a bid to cut costs have been branded unacceptable.
It emerged that NHS bosses are set to decide next month whether to shut four of Scotland's eight hospital laundry services.
The move would save an estimated £2.7 million a year, but Scottish Labour warned that it would be a mistake when hospitals were already running short on clean bedding and sheets.
NHS Greater Glasgow and Clyde alone 31 reported incidences of linen shortages in the last three years.
The eight laundries are based at: Aberdeen Royal Infirmary, Raigmore Hospital in Inverness, Borders General Hospital, Ninewells Hospital in Dundee, St John's Hospital in Livingston, Victoria Hospital in Kirkcaldy, the West of Scotland Laundry in Wishaw, and Hillington Laundry in Glasgow.
In the past two years, 27 jobs have been cut across the NHS laundry service - equivalent to five per cent of the workforce.
Scottish Labour health spokeswoman Monica Lennon said: Further cuts to NHS laundry services will be unacceptable.
The Health Secretary should come clean over this move and say how many staff are facing redundancies and what she will do to respond to the risk of industrial action.
Across Scotland we’ve seen hospitals running out of clean bed linen and towels too often, as staffing levels go down and health boards struggle to balance the books.
“After 12 years in charge, the SNP’s mismanagement of the health service is having serious consequences for patients and NHS staff and health boards are warning they’ll struggle to cope this winter with rising demand.
“Scottish Labour will challenge these proposals and seek a commitment that patient safety and NHS jobs are not at risk.”
A Scottish Government spokesman said: We understand that NHS National Services Scotland's Laundry Programme Board is seeking to develop a new action plan to ensure their services are as safe, efficient and sustainable as possible.
No proposals relating to this have come to Scottish Government ministers to consider.
We are clear in our expectation that any proposals that come forward must be developed in partnership with staffside representatives.

NHS worker diagnosed with Ebola in Glasgow
The first case of Ebola diagnosed within the UK has been confirmed in Scotland.
An NHS employee who had been working with Save the Children and returned from Sierra Leone last night is being treated in Glasgow.
The Scottish government says the risk to the public is negligible.

Tour Scotland wee video of old photographs of Langside, Scottish Gaelic: An Leathad Fada, meaning the Long Hill, a district in the Scottish city of Glasgow. It is situated south of the River Clyde, and lies east of Shawlands, south of Queens Park, west of Cathcart and north of Newlands. The original village of Langside was based around what is now Algie Street, named after Glasgow Merchant Matthew Algie. There were two mills nearby on the White Cart river, a meal mill and a paper mill which dated back to the 17th century. In the early 19th century most of the inhabitants of the village were weavers although they also cultivated fruits and flowers. In the late 19th century as Glasgow expanded South during the rapid growth of the industrialisation in the city the area was built up with tenements, the area became part of the City of Glasgow in 1891 with the last of the original weaver's cottages being demolished in 1905. The area was served by trams from 1901. The Glasgow Victoria Infirmary was a teaching hospital situated at Langside from 1880 until 2015. It provided a range of health care services including Accident and Emergency, General Medicine, including sub specialties, General Surgery, including sub specialties. Medicine for the Elderly, including Assessment, Rehabilitation, and Day Services, Orthopaedics and a wide range of outpatient clinics. In later years it had an acute operational bed complement of approximately 370. Queen's Park School in Langside was established in 1876 and in use until 1966. In 1568 the area was the site of the Battle of Langside, the last battle fought by the forces of Mary, Queen of Scots, prior to her exile and death in England.
