1. Segovia - the town with a Roman aqueduct, amazing cathedral and castle / alcazar makes for an easy 30 minute train ride or hour and a half bus ride from Madrid. Eat the suckling pig!
2. Toledo - the former capital of Spain and the seat of the Catholic church in Spain, Toledo offers cool streets to wander through, a great cathedral, a cool armaments museum & more.
3. El Escorial - the royal palace outside of Madrid is a cool way to see all the pomp and circumstance of the Spanish & European court.
4. Avila - this walled city let's you literally climb the walls and see a number of historic buildings such as their cathedral.

Ávila is sometimes called the Town of Stones and Saints, and it claims that it is one of the towns with the highest number of Romanesque and Gothic churches per capita in Spain. It has complete and prominent medieval town walls, built in the Romanesque style.
